10.02. The employer shall remit to the employee, at the same time as his wage, a pay slip containing the following information:(1) the employer’s name;
(2) the name of the employee;
(3) the employee’s hiring date;
(4) the employee’s classification;
(5) the date of payment;
(6) the work period corresponding to the payment;
(7) the number of hours paid at the regular rate;
(8) the number of paid overtime hours;
(9) paid holidays and annual vacations;
(10) the number of sick leave hours paid during the period;
(11) the total number of sick leave hours credits;
(12) the hourly wage rate;
(13) the gross wage;
(14) the nature and amount of deductions effected;
(15) the net wage paid to the employee;
(16) the amount of the employer’s contribution to the group retirement plan during the period and the total contribution during the calendar year;
(17) the amount of the employee’s volunteer contribution to the group retirement plan that was deducted by the employer during the period and the total contribution during the calendar year.
